Consular services will be provided in Phoenix, Arizona in the beginning of March
Consulate General of the Republic of Lithuania in Los Angeles is organizing a Consular Mission to Phoenix, Arizona on March 3-4, 2022.
Consular services will be provided at: 4409 E Montecito Ave, Phoenix, AZ 85018.
Appointment times:
March 3rd, from 8:00 AM to 12:00 PM and from 1:00 PM to 5:00 PM
March 4th, from 8:00 AM to 12:00 PM
During the Consular Mission Lithuanian citizens will be able to apply for passport and/or ID card renewal or issuance, submit paperwork for registering civil acts (e. g. child birth, marriage, divorce, death) in Lithuania, submit declarations of the place of residence, receive notary and other consular services.
Registration is obligatory. To register, please email a filled out form to cons.losangeles@mfa.lt by February 27th the latest.
Please note. During the Consular Mission all the consular fees are increased by 50%. Payment is only available by credit/debit card (except American Express).
For ordering a passport/ID card 2 photos will be needed (similar to U.S. passport-size, detailed requirements). Everyone over 2 year old will need to attend their appointment. To receive a passport by mail, the applicant must present a prepaid self-addressed envelope with tracking (USPS Priority/USPS Priority Express/UPS. Except FedEx). When ordering an ID card the applicant must present 2 prepaid self-addressed envelopes with a tracking.
